The philosophy of our practice is based on the concept of Preventive Dentistry. Our ultimate goal is to preserve your natural teeth for life and maintain your oral health at an optimum level.
It is our desire to meet your dental health needs as thoroughly and comfortably as possible. Our initial examination is used to obtain a complete dental history. In order to accomplish this we ask that you obtain the information on our "New Patient Previous Treatment" form for each family member that is new to our office and bring it with you to the first appointment. When calling your previous office for this information, please also request your current x-rays to be sent to our clinic. Having this information available at your initial appointment will help our staff formulate a more comprehensive treatment plan for you.
Below you will also find a patient registration form, health history, notice of privacy practices, and patient/clinic information. We ask that you complete and bring to your appointment the patient information, and a health history for each new family member.
